NorthLakes Community Clinic, Filling Gaps in HealthCare

 

NorthLakes Community Clinic (NLCC) in Turtle Lake is a Federally Qualified Community Health Center that operates 20 clinics in 19 communities throughout 13 counties in Wisconsin.

NLCC provides services based on the needs of the communities served. In Turtle Lake, they offer Behavioral Health, Mental Health Counseling, Chiropractic, DOT Physicals, X-ray, Dental, Patient Support, Health Insurance Enrollment, Sliding Fee Scale Assistance, Social Support, Recovery, and Substance Use Disorder Counseling.

All people are welcome to receive care regardless of their insurance status and ability to pay. For those who qualify, we have a Sliding Fee Scale based on income.

NorthLakes is deeply involved in the communities they serve. By providing Dental and Behavioral Health, dental care to nursing homes, and working with Women, Infants, and Children (WIC), they aim to fill the gaps in healthcare and fulfill the vision of healthy, prosperous, engaged communities where everyone thrives.

NLCC is committed to addressing barriers that hinder a person’s overall wellness by connecting individuals to services, resources, and other social supports so they can focus on their well-being.

Recognizing that financial cost is one of the most significant barriers to accessing healthcare, NLCC has a team of dedicated folks committed to helping find a way to make care more affordable. If you are uninsured or underinsured, this team can help you apply for Medicaid (BadgerCare) or navigate signing up for health insurance through the Marketplace. The Patient Financial Advocates (PFA) can also see if you qualify for a Sliding Fee Scale, which provides many services at reduced or no cost. PFA can also provide access to prescription assistance programs and other resources.

At Turtle Lake, a Community Health Worker (CHW) assists patients in achieving their wellness goals by identifying risks, services, and resources to support their whole health journey. Barriers include transportation, food, housing, safety, and social isolation. By working together, they help find ways to reduce these barriers. If needed, connections are made for additional healthcare services if not available at NorthLakes. These services are free to patients and available via telephone, video, and in-person at multiple clinic locations throughout their service area.

Another barrier to health is the high cost of fresh vegetables. In the past, NCC has received a grant from NACF to assist with the Farm to Patient Program. This program helps patients achieve wellness goals by providing access to healthy and locally grown foods. Participants receive a fresh box of produce seasonally. They receive nutrition education, recipes, and other support for healthy eating and lifestyle changes.
